We accept all PPO plans but do not accept any HMO plans or medicare/medicaid. Since each insurance provider has many different plans and procedures, please call us and speak with our Office Manager to confirm your coverage and co-payment details.

We are in-network with the insurance companies listed below. However, even if you do not see your insurance company listed, you may still qualify for out-of-network benefits. We are happy to help you obtain the benefits you’re entitled to and facilitate the care you require.

Payment Policies

For your convenience, we accept Visa, MasterCard, Discover and Amex, in addition to cash and checks. Please remember that you are responsible for all fees charged by the office, regardless of your insurance coverage. Payment is due at time of service.

Financing and payment plans are available for qualified patients through Care Credit, a personal line of credit for healthcare treatments and procedures. If paid in full by the end of the agreement period (6 or 12 months), this financing is interest-free. Please speak to our Office Manager for an application or visit carecredit.com for more information.
